The Add Site Visit Report Template form allows the identification and classification of the template that is being added. The Add Site Visit Report Template form contains the Template Name field and Visit Type field.


The Add Site Visit Report Template form is labeled Add Site Visit Report Template for a new record and Edit Site Visit Report Template for an existing record.

 

Template Name Field

The Template Name field is a text field used to identify the template within managers and on the Monitoring Visits form.
By default, the Template Name field is a required field and a black alert will fire if the Template Name data is not provided. The form cannot be saved until the Template Name data is entered.

 

Visit Type Field

The Visit Type field is a dropdown select field used to identify the type of template being added. This field will determine the templates that appear in each monitoring type on the Monitoring Visits Plan form [Link to]. These options can be configured via the Monitoring Type Managed List found in CTMS Settings. By default the Monitoring Type Managed List options are Evaluation, Initiation, Interim and Close-out.
By default, the Visit Type field is a required field and a black alert will fire if the Visit Type data is not provided. The form cannot be saved until the Visit Type data is entered.

 

Next Actions

Next Button

When the Next button is selected the data entered for the questions will be saved, a new template record will be recorded and the user will be directed to the Edit Monitoring Template Configuration form.

Cancel Button

When the Cancel button is selected the data entered for the questions will not be saved and the user will be directed to the Monitoring Template Manager screen.
