This guide is designed to assist you in completing a Fountayn User Access Request Form (UARF) and also to provide guidance on handling unique situations that arise when managing users. We are happy to help if you need any additional assistance.

D | Last Name | D - The user's last or surname E - The user's first name. Required fields for all requests. User's names (first and last) are often transposed. Please ensure they are in the correct fields. Certain characters will cause an issue when trying to upload a new user and should be avoided. These are:

User with Multiple Accounts |
If you find that a user has more than one account and you wish to have them combined, you can do so using the following steps:
Step | Action | Description |
1 | Locate All Accounts | Locate all of the user's accounts. Ensure that they do indeed belong to the same user. |
2 | Determine Account to Use | This determination is made based on the age of the account, whether it is active or inactive, and what - if any - training has been completed. Generally you want to use an established account. A user will be forced to complete any Prerequisite training that has not been previously done. It is important to note that access could be lost until all training is completed. Training can be manually credited to an account if necessary but it is best to do so using the oldest account. If this becomes necessary, please contact Fountayn University for assistance. |
3 | Assign Access | Once the account to be used has been determined, assign all of the proper access to that account. The system will automatically send an email to the user notifying them that they have been added to a site with a particular role. |
4 | Remove Access | Remove the unused account(s) from all of the studies. |
5 | Notify the User | Notify the user that their accounts have been combined and provide them with the username of the active account. The user will not receive notification of the disabled username so this becomes an important step. |
