Summary

This article provides instructions on adding Dynamic Forms to casebooks.

Dynamic Forms are forms that can be created by the user as needed.  Common Dynamic Forms include the Adverse Events, Concomitant Medications, and Medical History forms.  The Dynamic Forms available depends on the configuration of the study.  Users may also have a range of permissions in regard to Dynamic Forms.  For example, a user with ‘Coordinator’ role access may be able to edit a Dynamic Form but not be able to delete an existing Form.

The following information provides instruction on adding Dynamic Forms to casebooks. All examples demonstrate adding Dynamic Forms using Adverse Event forms as a model.

Access Dynamic Forms

  1. From the Subject Manager table, access a subject’s casebook by clicking the ‘Subjects’ link.
  2. In the Form Navigation table for a particular subject, click the link to a dynamic form summary.

Add Dynamic Forms

  1. From the Dynamic Form summary page, click the ‘Add’ link located directly under the Subject Bar.
  2. Complete the new dynamic form.
  3. Once the form has been completed, click the ‘Save’ button at the bottom of the page.

Upon completion, the system will assign a name to the form based upon the data entered.
